Community Transitions is a program that helps eligible seniors & persons with disabilities transition out of a nursing home back into assisted independent living.

Many seniors placed in a nursing home or assisted living facility may be able to, with assistance, regain their independence and live on their own again.
Perhaps you work full time and have concerns about how your parents are managing while you’re not at home, or your aging loved ones have specific care needs. These are the exact situations we can help you with. The Community Transitions program aids in linking individuals to home and community based services to maintain optimal independence in the home.
