MI Choice Medicaid Waiver Program

The MI Choice Medicaid Waiver Program is a comprehensive, community-based program designed for Medicaid-eligible adults who need nursing home level of care. The goal of the Medicaid Waiver program is to offer alternatives to nursing home care.

If you want to live at home, an apartment, assisted living facility or adult foster care, are age 65 or older, or age 18 and older with disabilities, we can help you determine if you meet the eligibility requirements.  Don’t assume that you will not be eligible if you currently do not have Medicaid. A lot of things are taken under consideration, and its still possible you could qualify for the waiver program.

Case Management

Case management is a collaborative process of assessment, planning facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.

Case managers are healthcare professionals who serve as patient advocates to support, guide and coordinate care for patients, families and caregivers as they navigate their health and wellness journeys.
